General Forms
Foxfield's Fixture Card lists schedules and prices for its lessons and camp.
Release Forms
All students need to fill out a Student Release Form before
they can ride at Foxfield. If the student is a minor, a legal guardian will also have to sign it. Foxfield also
requires a completed Medical Release Form from riders who are
minors. The form, which must be signed by a parent or guardian, permits Foxfield to seek medical
care for an injured or ailing student.

Camp Forms
You can apply to attend Foxfield Camp by filling out the Camp Application, or to work as a counselor or dishwasher by filling out the Counselor or Dishwasher Applications. These forms are accepted only at certain
times of the year, though, so see the Camp Enrollment Page for more
information.
To view these forms, you'll need Adobe Acrobat Reader, Version 4 or later.. These versions will allow you to both read and fill out these forms online, though you will have to print them and return them to Foxfield on paper. The Acrobat Reader can be downloaded from Adobe's web site.
